Simultaneous Primary and Secondary Syphilis Associated with Syphilitic Alopecia and Folliculitis in an HIV Positive Patient

چکیده
Syphilis has been classically divided into three clinical stages. The chancre that appears at the site of inoculation is referred to as the primary stage. The secondary stage is characterized by systemic involvement and the appearance of a disseminated maculopapular rash. Cardiovascular, neurologic and gummatous lesions characterize tertiary syphilis. The simultaneous presentation of primary and secondary syphilis has been reported because the chancre can persist into the secondary stage, especially in HIV positive patients. Although syphilitic alopecia is a rare manifestation of secondary syphilis it is an important differential diagnosis in the evaluation of patients with patchy hair loss. This article aims to present the case of a 34-year-old HIV-positive male patient that presented simultaneous primary and secondary syphilis associated with syphilitic alopecia and acute bacterial folliculitis. *Corresponding author: Isabel Cristina Valente Duarte de Sousa, Specialty in Dermatology, Hospital ABC Santa Fe. Hair loss is not a common feature of secondary syphilis. There are two types of syphilitic alopecia: “symptomatic” type where hair loss is associated with other symptoms of secondary syphilis, and “essential” alopecia that is either patchy (“moth-eaten” type), diffuse pattern with a generalized thinning of the scalp hair, or a combination of both without any other mucocutaneous signs of syphili...
متن کاملSyphilitic alopecia in HIV infected homosexual men: case reports.
Hair loss is an uncommon manifestation of secondary syphilis. Its clinical manifestations include diffuse effluvium or patchy ‘moth-eaten’ pattern of alopecia. Although syphilitic alopecia is non-scarring hair loss often associated with other mucocutaneous lesions of secondary syphilis, it might well be the only presenting symptom of syphilis. Secondary syphilis was diagnosed in ten homosexual ...

Syphilitic alopecia (SA) is a rare skin manifestation in secondary syphilis and sometimes indistinguishable from other alopecia. SA is of 2 types, symptomatic and essential, and 3 clinical types, moth-eaten or patchy pattern, diffuse pattern and a combination of both. SA in our case indicated symptomatic SA with moth-eaten hairloss. A 46-year-old Japanese homosexual man experienced hairloss fol...
متن کاملDouble Morphology: Tertiary Syphilis and Acquired Immunodeficiency Syndrome—A Rare Association
Background Human immunodeficiency virus (HIV) and Treponema pallidum coinfection is relatively common and accounts for about 25% of primary and secondary syphilis. Tertiary syphilis in HIV-uninfected and HIV-infected patients is vanishingly rare. This is most likely due to early treatment of cases of primary and secondary syphilis. There is rapid progression to tertiary syphilis in HIV-infected...
متن کاملAcute Syphilitic Balanitis and Gross Penile Edema in an HIV- Infected Man
Syphilis presenting with balanitis and penile edema is rare, and in most cases has been considered to be a manifestation of primary syphilis. We report the case of an HIV-infected man who presented with penile edema and balanitis concomitant with an eruption on his buttocks, lower abdomen and thighs. Histologic examination and serology were diagnostic of syphilis, and the patient was successful...
